If you have been issued a Wisconsin Driver's License, you must provide this number on the registration application form. If you have not been issued a Wisconsin Driver's License, you may use the last four digits of your social security number. It is not acceptable to use the last four digits of your social security number simply because you do not have your WI Driver's License with you. Failure to provide a WI Driver's License number if you have been issued one will result in a delay of processing the application until the information is provided.
All voters wishing to register must provide a proof of residence when registering to vote. Acceptable proof of residence documents include: valid WI Driver's License, utility bill, bank statement, paycheck, real estate bill, residential lease document, or any other government-issued document including the voter's name and address.
